I really want to stop being lazy and increase my efforts so I don't fail. This I believe will lead me to be more successful which will lead me to better expectations and less anxiety. I think my laziness is coming from Low self-esteem and every time I try and put a little effort to make it right, something makes me keep putting less effort into my studies and feel worried that I will fail my exams.
I already know that simply worrying about failing my exams because of lack of effort, I will fail them, but I want to fix that mentality.

Habits take a little time to break. Start taking small steps, ~easy things to accomplish daily/weekly/montly, like that. Each time you reach a goal no matter how big or small, pat yourself on the back. Exercising--even just daily walks, or every other day--any kind of exercise makes you feel good about yourself. It literally gives you a rush increasing endorphines.
Journaling your goals/successes truly give you good feedback. And ea. time you jot down a goal or two, it sinks into your subconscious making it a LOT easier to reach all the things you want in your life
